Scuba Diving Medical Form

This page is should be helpful for divers figuring out if they need to get a physician’s signature prior to a scuba diving activity.  If you don’t know, please read it carefully.  It is also where you can find a copy of the SSI Scuba Diving Medical Form  Failure to pay attention to this form could result in not being able to dive, and loss of money paid to participate in the activity!

For certified scuba divers NOT participating in a certification course

We do not collect medical information in this case.  You do not need to print or sign the form.  We indirectly have the information we need because you are certified and completed the scuba diving medical or something similar when you received the certification that you are using to dive with us.  It may be worth you looking at the form to make sure your medical information has not changed significantly.  It is also worth noting that the form now includes a question about whether or not you have had COVID-19.  It is possible that someone recovered has lung scarring that could effect their ability to dive comfortably .

For first time divers (try dives or intro dives)

Yes, you have fill out the form.

  • I’ve looked at it and answered NO to all the questions.  You are done.  You can print it and fill it out now, or just do it when you get to the shop.
  • I’ve looked at it and have a YES a question.  You have to print the form and get a doctor to sign off for diving.  It is important because if you have a YES, and then show up at the dive shop and are unable to dive because of that answer, we will not provide a refund at that point.  We do provide refunds outside of the cancellation window (normally 48 hours prior).

For divers participating in a ANY scuba diving course

Yes, you have to fill out the form.  It doesn’t matter if it Open Water Scuba Diver course or Advanced Wreck Diving.  Everybody does it for every course.

Depending on your needs, he may be able to clear you over Facetime/Zoom.   As of this writing it was a $90 fee and $130 if you need an in person visit.  If this is something you need, you have to arrange it directly with Dr. Young’s office.  His information is provided here for your convenience only.
